DOG EMERGENCY INFORMATION    

This is so important! First, your dog will be called by his name, a major plus, especially during a time of crisis.  Second, your dog actually has a chance to be taken home by the people on your emergency call list.  If your accident occurred far away from home, you've spelled out your wishes.  Now even if you didn't have this information for the rescue workers, your dog would still be taken for emergency care, however, if your dog were uninjured he would be brought to the local shelter and held there until you could pick him up.

Emergency personnel get the dog out of the car faster and with less upset if there is a leash on the dog.  Otherwise, they have to get the metal noose and "catch" the dog--a trauma you want to avoid!  Dogs or puppies who are leash chewers can have their leads clipped to the outside of their crates, still a better plan then having no leash at all.
 

In the event that I am in an accident and cannot communicate my wishes to the EMT's, they will be able to read the information I have given.  I keep an envelope in the glove compartment of my cars that says (in big red letters) IN CASE OF EMERGENCY−DOG INFORMATION

Additionally, I tape an identical envelope to the crate that my dog is traveling in.

IF FOR ANY REASON THE ABOVE NAMED PEOPLE CANNOT TAKE MY DOG HOME WITH THEM, AND IF NO MEDICAL ATTENTION IS NEEDED FOR THE DOG, I GIVE PERMISSION FOR MY DOG TO BE TAKEN TO A LOCAL BOARDING KENNEL.   I WILL PAY FOR ANY-AND-ALL FEES INCURRED AT THAT KENNEL. IF, FOR ANY REASON THIS DOG NEEDS EMERGENCY MEDICAL OR BOARDING CARE, I AGREE TO PAY ANY-AND-ALL FEES INCURRED BY ANY VET.
